Federal investigators are looking into two cases in which mental health patients were neglected at a Veterans Affairs facility in Brockton.

In one case, a veteran admitted in 2003 didn't have a comprehensive psychiatric evaluation until 2011, eight years later. In the other case, a veteran who lived at the facility for eight years had just one psychiatric note in his chart for that entire time.

David Farrell, the veterans services officer for the city of Brockton, joined WBUR's Morning Edition to discuss the cases.

This segment aired on June 24, 2014.
