Memorial for Milena del Valle

The ceiling collapse that killed a Jamaica Plain woman last year could have been avoided. That's according to findings from the National Transportation Safety Board.

Its investigation blames Big Dig contractors and the Massachusetts Turnpike Authority for negligence in the design, construction, and inspections of the tunnel system.

The NTSB released its report a year after Milena del Valle's death. Her family and friends marked the anniversary with a memorial service at a church in Jamaica Plain. WBUR's Bianca Vazquez Toness was there and has this report.
