Massachusetts is making changes to its screening process for foster parents. That's in response to the death of a 2-year-old girl in a foster home in Auburn and the near death of a second child.

The Department of Children and Families has just completed its investigation into what happened.

WBUR's Fred Thys outlined the agency's findings.

This segment aired on October 2, 2015.
