Who's To Blame For Recent Drug Lab Scandals?

Lawmakers held the first of three public hearings into two major breakdowns in oversight at the Department of Public Health two weeks ago. The first hearing focused on the national meningitis outbreak, responsible for 36 deaths and more than 500 infections across 19 states.

Tuesday's hearing examined the practices, policies and procedures which allowed state chemist Annie Dookhan to allegedly compromise tens of thousands of drug cases.

Are the two scandals — which occurred back-to-back — simply coincidence? Or is there a wider issue at play?
